How to read these fields

The board defines the two flags itself. On “Public Complaint”:

A public complaint (administrative complaint) is not disciplinary action. It is a charging document that has not been proven. A public administrative complaint is the result of a complaint and the investigation of the allegation of a violation of the practice act that governs a health care profession. The administrative complaint lists the violations of law that were alleged after the investigation and review that resulted in a finding of probable cause.

And on how a complaint becomes “Discipline on File”:

Some public administrative complaints are dismissed at the hearing stage. Other administrative complaints result in disciplinary action imposed by a final order, which is reflected as a “YES” in the “Discipline on File” entry on the licensee verification screen.

Not on the FDA's 503B list

The FDA publishes the complete list of outsourcing facilities registered under section 503B, with each one's inspection record. Brooksville Pharmaceuticals is not on it as of 2026-10-13. That is not a finding: most compounding pharmacies are 503A pharmacies licensed by their state and never appear on the FDA list. It does mean the FDA's inspection columns that this site shows for 503B facilities do not exist for this pharmacy; the Florida board's permit record above is the public record. 503A vs 503B, in the statute's words explains the two.

What this page does not tell you

Whether a specific drug from this pharmacy is right for you; whether the telehealth brand that sold it is the same entity; and what kind of permit each number is, because Florida's verification data does not say. Compounded prescriptions are labelled with the pharmacy that made them: compare that label to the trade names on the permits above.

Before you sign

Is this the name on your label?

If it isn't, look that one up. If it is a 503B facility, the FDA's inspection record is here; if it is state-licensed, we point you to the board.

Free · independent reference · no account.

Three checks before you sign

  1. Find the pharmacy on the label. Compounded drugs name the pharmacy that made them, not the telehealth brand that sold them. That name is what you look up.

  2. Read the public record. Whether it is an FDA-registered 503B facility or a state-licensed 503A pharmacy, when it was last inspected, and what the FDA wrote down.

  3. Compare the approved path. What the FDA-approved version costs today next to the compounded one, and which licensed providers prescribe it — so the choice is yours, made on the record.
